I can point out three different good starting point:
1) GUIDeS project - http://www.regione.emilia-romagna.it/guides
You can download the work that talks about what the process of
setting up a CA involves (problems, questions, etc... )
2) IETF rfc - http://www.ietf.org/html.charters/pkix-charter.html
There are many docs you may find interesting, take a close look
to the "Internet X.509 Public Key Infrastructure"
3) Take a look at the EuroPKI Certification Chain - www.europki.org
