I'm a little bit surprised because I think that you use 0.9.1-RC4 and
this RC uses a defaultoption which set the subjectAltName automatically
if it was not set. I will check the code again.

You can download the certificate via the public interface too.

Yes, there is an OpenCA-guide which is not really complete but it is a
good starting point.
