Dear all,

Below the shortest way I can think of, to make clear when to use what
Type of Archetype, also called by me a Documenting Pattern.

The archetypical (ideal) health business process model and
documentation patterns
Based on observations leading to evaluations the healthcare provider
tries to improve the state in the patient system via assumptions
(personal opinions) about processes in the patient system.
He operationalizes this via Instructions that are intended to lead to
planned Actions.
Actions can happen on their own as well - Unplanned Actions. (Acts of
God, Earth quakes, (road) accidents, lightning, etc, etc)

Documentation patterns
In EHR's healthcare providers document the process of the provision of
healthcare for a particular patient system.
The patient system is the patient as a whole and can include its
context (physical or social).
In the patient system processes change the state of thing in the
patient system all the time.
There are (at least) four distinct types of archetypes, types of
Documentation Patterns that can be used to document the provision of
healthcare.

- To document something about the state of something in the patient
system, at point(s) in time in the past, using a specified method, we
have to use the Observation Documenting Pattern.
- To document something about a process in the patient system at this
point in time, we have to use the Evaluation Documenting Pattern.
- To document something that is intended to interact with the patient
system, at a future time, by issuing orders for Actions, we have to
use the Instruction Documenting Pattern.
- To document something that has changed the state of the patient
system, at a point(s) in time in the past, we have to use the Action
Documenting Pattern.

The key elements are in bold.
