default value definitions must be ANDed

Bug description:
the default values for journals gives 3 conditions
* company_id
* partner_id
* invoice_type
in the saved records (ir_value) partner_id and invoice_type are in
different records and AFAICS these are not related
IMHO it must be possible to define values for partner AND type not
only for partner OR type
No idea what the results are in the case that different values are
defined for partner and type (probably random)
