I really don't know enough about v3 to answer that.  The vast majority of my
HL7 work is with v2.  The GP2GP project
(http://www.nhsia.nhs.uk/gp2gp/pages/default.asp) might be a good place to
look as they are trying to do just that.

Yes, but again I'm not the best person to ask.  HL7's Java SIG is working on
an open-source v3 library (including data types, information model, and XML
serialization) but I don't think anything has been released.  There are
certainly projects here and there using v3.  Aside from the GP2GP project,
another one that comes to mind is Canada's National e-Claims Standard.  I'm
not terribly familiar with these projects, just aware of them.
