There is an effort spearheaded by the AAFP to create a standard for the
content of a primary care record, the CCR.  I've been in contact with
David Kibbe, MD, the "EHR Centrality" for the AAFP, who is wondering how
they can collaborate with the open-source EHR community.

The ASTM CCR is a collaboration of the AAFP, the Massachusetts Medical
Society, the American Medical Association, the American Academy of
Pediatrics, the Health Information Management and Systems Society, and
the American Health Care Association, (but not my guys, the ACP) with
the standards-development organization ASTM International.

The CCR effort is summarized by the American Academy of Family Practice

To get a sense of the purpose and design of the ASTM CCR, you can take a
look at the application at www.continuityofcarerecord.info (ID=doqit and
password=demo).  This is a CCR creator or editor built by Ken Miller of
Solventus.

I am confident that the project would welcome suggestions.
