Resolution: Cannot Reproduce
Sorry, I can't reproduce that, it seems like a file could not be loaded, I
actually implemented this dialog to have at least the URL, so you can simply
copy the URL and then check what file exactly was requested. As long as the SID
is active you should be able to reproduce it.
