It can be difficult to reproduce, but if I persist (or sometimes by accident), it appears. This remains on the screen even if I log out. I believe it has to do with the events for the tool tips, if I move back and across the tools a few times, and allow the tool tips to change, then this issue with sometimes happen. Maybe it is with on specific tool, but so far I have not been able to identify a specific tool that causes this.
