I also met the problem in our lab, and my question is how to recovery when
issue occur without reboot the node?
After I reboot the node, the issue gone.

It means that your application process related to 'mgt-a.mca' has exited or
crashed. I.e. the registered process associated with the 'mgt-a.mca' component
has exited/crashed.
'ava' refers to the 'AvailabilityAgent', i.e. the SAF AvailabilityAgent library
that your application component/process has linked with.
