hmm, on our prism I don't see any of these defines. Could you run 

rpm -qf /usr/include/GL/glext.h

and send me the output. As I haven't updated our machine for quite a
while it might be outdated. BTW is yours running RHEL or SUSE ?
And it would not be the first time a patch screws some headers.

yes, I'm about to commit the fix. As FTGL uses a GLId there was some
code missing in order to make the cluster run.
