I don't think a "good package management system" is enough.  I actually
don't think there is a solution to this "problem."  The reality is that
there are thousands of people from many different cultures working on
the products.  Even in well-planned cities (eg. Washington, DC) enough
cruft gets added with thousands of interdependencies that getting from
point A to point B may be a circuitous route.  As the city grows to
accomodate more citizens, it becomes more random, not less.  Why should
we expect software to be any different?
