I would suggest something like http://www.surveymonkey.com/ to run the
survey and capture the results. They have a free version but I think
you are limited to 10 questions and 100 responses per survey. If we
want to start doing more things like this it might be a good idea to
post a privacy policy on the site since it would be beneficial to send
this out to people subscribed to the mailing lists but wouldn't want
to surprise people with unsolicited email.
