We store gigs of images as BLOBs in our production databases in our
transaction tbls 
with the DISABLE STORAGE IN ROW clause - causing the image to be stored
outside the row.

For me it's more a question of performance than
fragmentation. 

At my last client (www.houra.fr), the images were
store at the OS level to speed up the showing of the
web pages and in Oracle so that each managers could
associate comments to the images.
