Are you looking for something trivial like:

select n.first_name, 'E.' middle_initial,n.last_name,m.mem_init_sequence
from   names n, member m
where  n.first_init=m.mem_first_init and
        n.second_init=m.mem_second_init
order by 1 desc, 2 asc;
