I do not apply any one-off patch [except for Security patches] unless I have actually encountered a situation where I need it or the application requires it [e.g Apps R11 on 8.1.7.2 and 8.1.7.4 requires additional one-off patches].
The one-off patches are not "regression-tested".
Security Patches must be applied [there's a seperate IT Security group and these guys keep a watch on bugtraq , SANS etc and other alerts. They don't see http://otn.oracle.com/security but do ask me about patches notified by SANS]
