A few possibilities come to mind:
shutdown abort
user shuts off PC without logging off
user kills app without logging off
disgruntled DBA uses 'alter system kill session'
oracle crashes
server crashes
probably a few more

Subject: DBA_AUDIT_TRAIL: some logon session have no logoff
DBA_AUDIT_TRAIL: some logon session have no logoff
I have several entries in the dba_audit_trail that
have ACTION_NAME=LOGON but the corresponding
LOGOFF_TIME is NULL. The dba_audit_trail has exactly 1
row for that sessionid.
The issue is that these entries are several days (5-6)
old, and there is no corresponding session in
v$session (AUDSID). What happened to these (LOGON)
sessions? Is it that they are logged-off by the system
based on idle-time and do not get an audit entry ?
