Here's one more addition to the "smallest orchids" category:  Notylia mirabilis (synonym Notylia norae) from Venezuela.  It is described and photographically illustrated in G. C. K. Dunsterville's Introduction to the World of Orchids, and also in his Orchid Hunting in the Lost World (and Elsewhere in Venezuela).  The latter is a collection of the many articles that Dunsterville wrote for the A. O.  S. Bulletin over many decades.  There's an excellent photo of N. mirabilis on page 120.  The entire plant, including inflorescence  is photographed next to a thimble (as it is in Introduction to the World of Orchids), which looks massive compared to the plant.
