Actually it is not really a problem: for SystemImager on Debian, we (to be 
honest more Dann than me) apply few patches and it does not create any major 
problems. Typically i think the way to deal with that case it to create a new 
package. Let's say the package is foo-2.3.1 and you found a bug, you have to 
apply a patch. It is pretty simple to create a new "version" foo-2.3.1-gv1 
that includes the patch. Of course, you have then to discuss with foo 
developers in order to integrate the patch directly into the source.
Anyway, just long sentences to say that this case is not a problem for me 
currently. :-)
