I have led OST with youth including primary school. I've not had such bullying happen. With that much bullying, I would certainly ask myself and the sponsor about the theme question as has been suggested and the context set by the sponsor at the beginning of the OST session. How did formal power set the context? Was any framework established. I would think about the "container" that was created for the conversations. Certainly the theme question is crucial, but also the clear articulation of the context, the principles and law and what will happen with the results of the conversation all help to create a "container" for focused and "healthy" conversation. Self-organization happens. For me, OST is about creating a "container" (socially constructed) for focused, "healthy" and productive self-organization and for learning from "whatever happens". The sponsor is more important than the facilitator in setting this context.

And in the meeting itself. . . Trust the people. They will work things out, given the space/time to do so. And what about the young lady who was ejected from her own group? Needless to say I wasn't there and don't know the situation. But a plausible story could be that she was a bossy, strong minded young person who knew exactly how the issue she raised must be discussed. As it happened the other folks in the group had different ideas and also really wanted to discuss the issue. Ergo.
