instrument now published as a kit for therapists (see
www.lifecoursepublishing) and 2) a book of forms for people with sensory
impairments to bring to their appointments with health care providers: it
provides a place to record medications, visits with various physicians, and
allows the physician to keep a copy. However, though I have a PH.D. in
