Pedretti's _Occupational Therapy: practice skills for physical dysfunction_, ch.26, p.483 has step-by-step pictures for a button up shirt. This book was an OT school standard issue reference... I recently dug it out needing this same info. Researched on-line as well, but found the book to be more helpful.
