You wrote, that you are using RH. In your apache configuration I saw, 
that the grup and user for the webserver is apache. With which arguments 
did you execute the SetPermission.sh script? IMHO you have to use:

./SetPermissions.sh /opt/otrs otrs apache users apache

Are this directorys right?
