If you like to set the permissions this way, you should use

SetPermissions.sh /opt/otrs otrs >webserveruser> >otrsgroup> www

Replace <otrsgroup> and <webserveruser> with the right values of your 
system and put the otrs user also into the webserver group.

Please tell me:

- How is your otrs user called (I think otrs)?
- In which group is your otrs user (I think user)?
- Under which user runs your webserver (I think www)?
- In which group is your webserver user? Thats still not clear to me :(.

Please let me see your apache configuration and your Config.pm from the
$OTRSHOME/Kernel directory.
