This is an SQL error.
Did you grant permissions on the SQL database (the one for OTRS)?
i.e.:
mysqladmin -u root -p 'grant all on otrs.* to [EMAIL PROTECTED]
identified by "some password" '
SQL maintains its own authentication system.
