Carlos is correct, this happens when you install otrs in another directory other than /opt/otrs. Two things must happen.Hi Andrew, Did you set-up the The "Core:Web" at "Framework" at "Sysconfig" ?You must specify the "FrontEnd::ImagePath" parameter acknowledge the icons.
1. You must have an alias for this configured in your web server2. If the alias is not /otrs-web/ or if your base directory is not /<alias>/images/Standard/ then this must be configured manually in the Sysconfig.

Do you have empty place holders for the images in your browser? If you right-click one of those you can check the path to the picture. Check on the server that it does indeed exist.
