Thanks for your reply.
I only have one queue and all agents have rw rights on it. If I maually modify 
the TicketCounter.log, the system picks a few tickets then again conks out. 
What's a more permanent solution? My setup is live and serves 250 internal 
users. The 2nd instance (which does not have the error) is still stable and 
serves all public queries.

This error could bee because
you are trying to create a ticket in a queue with out rights. For example:
 
You have a queue 1 and insight
it the queue 1-1, in the queue 1 you have read rights (or no rights) and in the
queue 1-1 you have rw rights.  If you try to create the ticket in the queue 1-1
everything works fine but if you try to create the ticket in the queue 1 you
receive this error in the log and the ticket is missing. If you have this issue
give enough rights to the parent queue.

I have two instances of otrs running under the same environment; the other one
