I'm not an expert but I think OTRS code should do something like this:

# Standard DBI connection.
my $dbh = DBI->connect("DBI:mysql:${database}:${hostname}", $username,
$password) 
    or die "DB Connection not made: $DBI::errstr";

$dbh->{'mysql_enable_utf8'} = 1;

$dbh->do('SET NAMES utf8');
