I have never been known for being subtle on the topic of women’s rights for vbac! ;o)  Misinformation is our biggest hurdle and people need to get educated and informed and REALLY critique the research what ever it says! 

 

Interestingly the SA Perinatal guidelines state that the continuity of care of a known midwife is appropriate care for vbac...coupled with a whole heap of other fear mongering related stuff.

I agree what ever  the rates he quotes or not,  for ruptures with repeat  C/s then he & his organisation should/needs to be asked why he/they are not making every known effort (for example supporting 1-2-1 midwifery services)  especaily for those who have had 1 C/S  or more to lower the rate of any repeat C/S!

To my knowledge there is no evidence to prove that rupture rates exceed about 1-3% with two or more cs.   He is talking shit David (as if we didn’t know) and you could argue that if DM’s rate of rupture is between the 5-20% then HE should be reported for endangering the child’s life.  I will send all the studies I can find today (well the details of them anyway) but have to do the school thing first.

Recently Dr David Molloy (President QLD AMA) was quoted as saying that "the risk of uterine rupture in a woman attempting VBAC is between five and twenty percent" (The Australian 4 Feb 2005). My research could only find scientific evidence showing rupture rates of between 0.3% (Henci Goer - Obstetric Myths and Research Realities, 1995, 42) and 2.45% (Lyndon-Rochelle, Holt, Easterling & Martin in the New England Journal of Medicine - 'Uterine Rupture During Labour Among Women with a Prior Caesarean Delivery 2001 v345:3-8) and that upper rate was only for prostaglandin induced labour.

I wrote to Dr Molloy asking him for a reference to his research evidence for the five to twenty percent rupture rate. Today I receive his reply. It said simply: "This figure relates to a woman who has had two previous caesarean sections."

He provided no reference for his figure.

My question to the wise people on this forum, is can anyone tell me the reference for Dr Molloy's assertion?

I have written to him again seeking a reference, but as I am keen to look at the evidence myself shortly, I can't wait.
