Not a tool to do this per se, but we use a HTTP Redirection module to redirect the old links to the new ones, and then *encourage* the content maintainers to fix these links up as soon as they can. Remember, links exist in a lot more places then just the page content - word documents, emails, flash training courses, users favourites, etc. etc. Of course, this means someone has to maintain the redirection mappings.....
