Have you checked all the paths in the theme... you cannot really use relative paths because you don't know how deep the site will be in the site structure.
i.e. having an image like this: <img src="..\images\myimage.gif"> won't work when the site is more than one level down. In this site I have a site collection based on team site template but this one does not show the theme. If I see the settings of team site, it shows custom theme as selected but does not apply it.
