We have recently setup a similar solution that uses FBA.

The administrator creates the users via a custom list form. Users get added
to the site's Reader group and also to a Users list which includes further
information. One of these is a custom field which captures the Last Login
time.

I am now in the process of taking this a step further by allowing users to
self-register to take the load off administrators. They then simply approve
the account as required, which triggers a workflow with email action.

I'm also hoping to add additional fields such as an auto-increment field to
capture the number of times a user has logged in.
