Not sure you can do this with a list, but you can do this using a Forms Library. Publish the form template to Central Admin. This will create a content type that you can then use in the forms libraries at each of the subsites.
When you republish the form template, it will get pushed out to the relevant libraries. Trying to retrofit this to an existing set of lists sounds scary. Long live InfoPath!
