Yes the database is running (command used to check this "service mysqld status")

I think I sorted out the database issue by upgrading the pfschema.mysql to 4.3.0

I'm still getting the fix permission error from web gui. I can make changes in 
config files from CLI.

Any other suggestion is much appreciated.

Then you likely have a database issue and not a permissions issue.

Is the database running?
Can pf connect to it?
If the database is missing columns then something is very wrong.

What release of PF is this?
On which OS/distro/release?
