Can you please post the contents of your conf/authentication.conf and 
conf/profiles.conf files? 

There are conditions under which it may not make sense to use a password.
The display (or lack thereof) depends on the value of a “no_password” variable 
which you may find at line 75 of html/captive-portal/templates/login.html (for 
example).

You could alway edit that template until you figure out what is triggering that 
condition.
