This is the same issue I have been struggling with for a couple of weeks 
now. Here's the upshot of what I've figured out, so far:

RealPlayer can NOT be set to play through the secondary card. Whichever of 
your two cards you decide to make the primary, that is the one RealPlayer 
will have to play through. However, you CAN reassign JAWS (or, more, 
precisely, eloquence) to the secondary card. Still, the Windows system 
sounds (if you use them) also cannot be reassigned to the secondary card; 
they, too, must play through the primary. So, you will have to have your 
Windows system sounds AND RealPlayer playing through the same card (the 
primary), and you can reassign JAWS to the other card. If such a 
configuration will meet your needs, let me know, and I'll let you know how 
to implement it.

Personally, I've been trying to get Realplayer and Winamp to play through 
one card, and JAWS and the system sounds through the other. There doesn't 
seem to be any way to accomplish this, as Realplayer and the system sounds 
both have to play through the default card, whichever one that is.
