Sounds like some of it maybe it's time for a new PC. As for track 25. Could it be that the CD is too roughed up at that section where that track is written? Is the HDD makeingg any funny noises? Could the empty folder thing be doe to some Hard Disk problem? That's the only time I've had that happen.
