Composite video renders a grey screen.   I think this
is a hardware issue, as I have software troubleshooted
already.  I checked that all a/v extension are
enabled, maxed VRAM, booted with TV only.  I always
get the same grey screen that changes size with a
resolution change.

Unless you set the desktop pattern you would likely get a grey screen. Have you tried moving the cursor or a window around the other screen. Try dragging the cursor along the length of each edge of the main screen, looking for it to pop into the RCA display.
