What do I need to connect the modem port on my 9600 to a phone jack? Is
there a 'patch cord' with the correct plug on each end? If so, what is the Apple
part number and where can I get one?

That is only a connector for a modem, not an actual built in modem. Apple labeled the ports "printer" (also for appletalk networking) and "modem" for regular serial connections, including an external modem.

You will need an external modem to connect to a phone line.
