Hey all,
I have a 9500 that's been upgraded with an XLR8 G4/366 ZIF and carrier card, 512MB EDO RAM (it's supposed to be compatible with G4 upgrades), a Sonnet Tempo ATA-100 PCI card, an IDE 40GB drive, a USB card, a Firewird card, an IDE Apple DVD drive, and an ATI Rage 128 graphics card.
I've been trying to install OS 9.1 on this thing and it's giving me a devil of a time. I bought a full install OS 9.1 CD from Apple Rescue and I'm getting weird behaviours from the OS when I'm either trying to install it or when it's installed. Sometimes the computer will NOT recognize the CD and it gives me the flashing question mark/floppy disc icon (I've used the option key on boot and I've also used the C key on boot). I've zapped the PRAM, I've pressed the CUDA button and the only way it seems to work is to use an older daughter processor card.
Next when I finally get the OS installed on my drive I've noticed that double clicking on the icons (both folders and apps) does nothing! I have to click on the folder or app and then go to file and choose open. Also the feature of just clicking on a menu heading and having the drop down stay open doesn't work. I have to hold down the mouse button and then scroll down to my choice ( sort of like "old school" Mac).
I don't think it's my CD since it installed fine on my B&W and I never saw the same weird behaviour and this was AFTER I had tried to use on my 9500.
Anyone have any ideas what is going on?

Rebuild the Desktop !
Is this a clean install or an overlay?
You may have some old files left that interfere with the OS. Do a FileBuddy search or Sherlock for duplicates.
Also disconnect the CD cable and reinsert it several times at both ends to break through any film of crud. Especially if from a smoky environment. Or use electrical contact cleaner. inspect the cable for small breaks also.
Do the same on the hard drive as the questionmark icon may indicate that need. Also are the drive ID numbers set right.or a termination missing. It is usually on the CD Do an ASP check.
My install of 9.1 takes a little while to start as I have a lot of extensions. I can't get it going for 30-40 seconds after the desktop assembles.
I.m just used to it.
Good Luck,
