I had this problem, but I fixed it by removing the following
extensions from the Extensions folder:  Authoring support, Disc Burner
Extension, USB Authoring support.  If you ever decide to use a
Firewire CD burner then you also need to remove Firewire Authoring
Support.

You didn't mention what version of OS 9 you are running.  From what
I've head using Toast 4 with OS 9.1 and up provides the most success.
