One way is to open the PDF in Acrobat, right-click on the image with the Touch-Up object tool and choose Edit Image. By default, providing you have the application, this will open Photoshop. Once in Photoshop, desaturate the image (Image > Adjustments > Desaturate) and save it (NOT Save As) and the image in the PDF will now be Black and White. You can even change the image mode in Photoshop to Grayscale and it will still work.
 
If you use an image editor other than Photoshop, you can change the default preference:
Edit > Preferences > TouchUp: Choose Image Editor.
