I'm not sure what version of Acrobat you have so I'll go with what I know which is 6.  
You can create a form manually using the form tools and then export the form data to a 
file which can then be imported to other pdfs duplicating the form.  Even if the forms 
are slightly different you can move the form fields around a bit after importing them 
which is faster than creating them from scratch.  This is the poor man's way.  There 
are 3rd party plug-ins and helper programs that create forms as well.

Does anyone know an easy way to make PDF forms interactive? I have a whole bunch of 
legal forms in PDF, it would be great if I could easily change the fields to be 
interactive so I could just type right onto it and fill in the check boxes, etc. 
 
Any suggestions?
