Jason - do you mean support for creation of tagged PDF or consumption/viewing or something else?
I ask because there ARE open source solutions that do generate and consume tagged PDF. Also, while not open source, Adobe Reader is available on many platforms for "free" and does exposed both tagged and untagged PDFs to screen readers including ATK (on Linux).
