I am the webmaster of a labor organization website and am looking to develop/introduce an interactive Acrobat .pdf form.  I am unsure if the principles of this form would work, however this is the purpose of this e-mail.  I am seeking some ideas and/or recommended software which would make this work.
 
I am looking for create a form (Letter) which would be accessed by members across the United States.  This letter would be a form letter, however the first page (or Fill form page) would request the author to check a box to indicate which state he/she was from.  This would then reference a separate list/file which names and addresses and import this information into the Addressee area.  As part of this 'question page' the author would also complete his/her information.  Upon clicking done or complete...the Form Letter would import the data and complete the letter. [We are looking at being able to create grass root letter to congressional representatives and import the Congressman/Senator information into the addressee area without the author having to type anything(except their personal information).  As Representatives change we could just update the Representative file.(.txt, or other data source)]
 
It would also be nice to incorporate this exact concept into our website for a (1) - (2) - (3) auto-form process...
 
Is there a better way? Format? Ideas?
