If reliability is a problem then I guess having "emailing forms" on the
web for general consumption may not be the best idea. All of our forms
are on the web but are used by employees for internal use only. After we
got everyone upgraded to at least Acrobat 5.1 we have had no problems
with this method of data collection, but again it was only with our
employees.

One reason for not doing that is Reader (normally) cannot send the
entire 
modified PDF via e-mail, or send an FDF directly via e-mail. It can
submit an 
FDF to a web server, however. You may be able to get an FDF sent for
some users 
via the mailto scheme, but it is very unreliable.
