Hello:

I have been trying to implement a personalized certificate on our
website, using the code from "Extending Acrobat Forms With Javascript"
(John Deubert, Adobe, 2003) as a starting point.  (Basically I'm passing
information to the PDF form that is then being displayed in the
appropriate fields using JavaScript.)  

This form is working perfectly for some people but not others, and
there seems to be neither rhyme nor reason for its misbehavior.  I have
tested it successfully in the following combinations:

* Mac OS 9, both IE and Netscape (versions unknown)
* PC Windows 2000 Professional, IE 6.0, Opera 6.01, and Netscape 4.78
* PC Windows XP Professional, IE 6.0
* PC Windows 95, IE 5.5

Failures have occurred in:

* Mac OS X (Jaguar), IE 5.2 and Safari 1.0
* PC Windows ME, IE (version unknown)

All of the above were running Acrobat Reader 5.0 or 5.1.  By failure, I
mean that all the form fields were blank.

If anyone has any suggestions about what might be wrong or how I might
troubleshoot further, I would greatly appreciate it.
