You can start the form by converting a MS word document into adobe.

Mirjam-

Use the form tool and define what type of field you want in the upper right
hand corner(Text, check box, etc.)  As for calculations, Acrobat 5 has a
built in function for addition, multiplication, average, minimum, and
maximum.  Division and subtraction have to be done in the custom calculation
script box under the "Calculate" tab.  Just drag your cursor over the tools
if you have them displayed at the top until you find the forms tool (F).

Hi,

I just start to work with adobe. 
I am a programmer for about 7 years in most of the program-languages. So i
can make the code of a form, but can somebody tell me how to start the form
in adobe. or how i can make a form without javascript? (I am working with
adobe acrobat 5.0 and filemaker)

I hope somebody can help me.
