If they have the full version of Acrobat, just export the form data to FDF or XFDF.  After that you will probably need a script or program that can read the format into a database or to a comma-delimited file...I have done a similar script and if you are interesting in custom development let me know...

I have a user who created a PDF form with form fields and no submission. They received back 25 responses and now they want to extract the data. Any suggestions?
