Gordon, the Adobe Document Server will allow you to assemble a multi-page PDF with form fields, and retain the field values without flattening.
Just got off the phone to Adobe here in the UK the opening version is $20,000.00 but you are quite correct in that it will do exactly what I want.
However as you may guess it is beyond the budget for this operation.
Good to know for the future though and thanks for the idea.
