[PDF-Forms] webform failing

Hello PDF Gurus!   I am new to the list and have been searching for an answer to this problem all day. I searched the archives, but nearly every link I attempted to go to that sounded like it may have had an answer appeared to be an old link (from 1999/2000).   So, here is my problem: I have a webform: http://www.chesapeakeendo.com/forms/MedicalHistoryfrm.pdf ...that launches a formmail.cgi script.   When submitted, I get this error message: Server error! The server encountered an internal error and was unable to complete your request. Error message: Premature end of script headers: FormMail.cgi If you think this is a server error, please contact the webmaster Error 500 ChesapeakeEndo.com Sat Oct 18 00:27:18 2003 Apache/2.0.40 (Red Hat Linux)   What might it be looking for?   Thanks a lot!
